It Starts with an Assessment
Our service starts with an assessment of your needs. We offer a FREE online Health Check or a more detailed Home Health Assessment carried out by a healthcare professional, in the comfort of your own home. We use your responses to understand your healthcare needs and everything you share with us is confidential.
Your Personalised Report
On completion of our FREE Health Check, you’ll receive a free, personalised report with advice on staying healthy and independent when you age.
If you choose to have our comprehensive one-to-one Home Health Assessment, you will receive a detailed report that helps you understand whether you are at risk of an unexpected fall, injury or illness. If you’d like further support to help you minimise any risks, Significare can help.
Supporting Healthy Ageing
Based on international best practice in age-related healthcare diagnosis and treatment, our unique assessment process, supported by our multidisciplinary team, recommends the treatment you need to stay healthy, mobile and independent.

Expert Care, Co-ordinated for You
We co-ordinate the support you need, including treatment planning, ongoing needs assessment and progress reviews.
Our services are provided in the comfort of your home.
Registered health and social care professionals deliver services including physiotherapy and occupational therapy, without long waiting lists. Care Navigators provide independent advice to support your personal choices. This ranges from guidance on care support, costs, legal issues and steps you may want to consider taking to support your changing needs.
We offer a range of paid-for treatment plans and you can return to the NHS for treatment at any time. We even co-ordinate your treatment plan for you, so you don’t have to find therapists or travel to clinics.
